Cancellation policy

At Mental Fitness Matters, we aim to provide mental healthcare access as quickly as possibly to as many patients as we can serve. Our high-quality, licensed clinicians are in high demand, and their time is carefully booked out in advance. Therefore, we have a strict cancellation policy to ensure our clinicians can manage their schedule, as their time is our most valuable commodity.

 

Cancellation Fees:

If you cancel your appointment less than forty-eight (48) hours prior to your scheduled appointment time, you will be charged a $110 cancellation fee. You may schedule a new appointment at any time. The only exception to this policy is if you provide a doctor's note.

 

No-Show/Within 24-hours of Appointment Time Cancellation Fees:

If you do not show up for your appointment and/or have not canceled more than twenty-four (24) hours prior to the scheduled time, you will be charged the cash-rate for your full appointment time. For a one-hour appointment, you will be charged $225 (this includes NEW patient intake appointments). For a forty-five minute appointment, you will be charged $195. For a thirty minute appointment, you will be charged $150. You may schedule a new appointment at any time however if you no-show three times, you will be subject to being discharged from the practice. The only exception to this policy is if you provide a doctor's note for your absence.

 

Note that the fees mentioned above are not reimbursable by insurance providers and will be charged to the credit card on file. All cancellation fees are subject to rules and restrictions that may be governed by any state or federal law.

DISCHARGE FROM THE PRACTICE

Discharge / Termination from Practice Reasons:

  • Ongoing pattern for missed or canceled appointments

  • Failure to follow the recommended treatment plan or medical instructions

  • Negative findings on the Prescription Drug Monitoring Program website

  • The provider cannot provide the level of care necessary to meet the client’s needs.

  • Inappropriate use of prescribed controlled substances

  • The provider is relocating outside the service area.

  • The Client and/or the Client’s family is abusive to the provider and/or practice staff, or poses a serious threat of harm to the provider, staff, and/or other patients.

* Please note there are other discharge reasons outside of the above list. We will assist with transition to a new practice if desired, and provide necessary care for 30 days once patient has been notified of discharge.
